  1. Sober Living Home Application Guide (PowerPoint) – A PowerPoint guide, provided by ADHS, offers step-by-step instructions on how to license a sober living home.

Sober Living Home Complaints and Enforcement Actions

  1. AZ Care Check: Sober Living Home Providers: AZ Care Check offers a searchable database that presents the licensing history, including any deficiencies or enforcement actions taken against facilities and providers.  

  2. Licensed Sober Living Homes Online Complaint Submittal Form:  ADHS offers an Online Complaint Form designed for individuals with concerns or knowledge about potential rule violations, enabling them to report their suspicions.
